Get a copy of Form DS-11, Passport Application Form, which can be obtained at any Passport Acceptance Facility or from the US Department of State Web site.
Fill out Form DS-11 with your personal information, including your name, address, phone number and social security number.
Visit any Miami Passport Acceptance Facility, such as the Clerk of Court at the South Dade Justice Center or the City of Miami building to submit your Passport Application, such as the Clerk of Court or the City of Miami office. Along with Form DS-11, submit your proof of citizenship, proof of identity and two passport photos. Acceptable forms of citizenship include birth certificates, Consular Report of Birth Abroad, Certificate of Citizenship and Naturalization Certificate. The US Department of State accepts Naturalization Certificates, driver's licenses and current government or military IDs as proof of identification. Two passport photos must be two inches by two inches, less than six months old, in color, between one and one and 3/8 of an inch from chin to forehead and taken in normal clothing wearing no headgear or non-prescription glasses and must be of your full face on a light background.
Submit your application and required documentation to your local Passport Acceptance Facility employee and pay the Passport Application Fee of $100. Your passport will arrive in the mail four to six weeks after your application is submitted. To receive your passport two to three weeks after submitting your application, tell the Passport Acceptance Facility employee you would like your passport expedited and pay an additional $60.
