Navigate to the Ohio Department of Health website. Click on the link in the Resources for "Vital Statistics."
Download a copy of the "Application for Certified Copies." This application is used for birth certificates, death certificates, still birth certificates, fetal deaths certificates, paternity affidavits and heirloom birth certificates.
Complete the application. Select "Birth Certificate" and provide the full name of the person for whom you are requesting the birth certificate, the date and address of her birth, the father's name, the mother's maiden name and any new name that is used by the person, if her name has changed. If any of this information is not known, then you can have the office search through the vital records for an additional $3 fee.
Submit the application with fee to the Office of Vital Statistics. The fee for a certified birth certificate is $21.50. You can mail the application to "Ohio Department of Health, Revenue Room, Vital Statistics, P.O. Box 15098, Columbus, OH 43215-0098." It may take 3 to 6 weeks to receive the certified copy of the birth certificate. You may also locate the health district office where the person was born and submit the application in person. To obtain the certified copy of the birth certificate on the same day that you make the application, take it to "Ohio Department of Health, Vital Statistics, 225 Neilston St., Columbus, OH 43215."
