Make sure you have proper identification. New Hampshire will only release a certified copy of a birth certificate to the person whose name is on the birth certificate or a member of the immediate family or other authorized person. You will need a valid driver’s license, state photo ID or other official photo ID. If you don’t have a valid photo ID, you can download the “Declaration Form” using the link below and fill it out. You will need to include this form and copies of two alternative documents from the list on this form (bank statements, utility bills and other items.)
Go to any town/city clerk if you live in the state and file your application. You will find a list of town clerks and contact information at the link below. The fee to get a copy of a birth certificate in New Hampshire through a town clerk is $12.00.
Mail your request if you live outside the state. Yu will need to download the application form using the link below. Complete the application and include a photocopy of an current photo ID or alternative documentation (see Step 1) and a check or money order for the total amount due (the Department of Vital Records does not accept cash).
