Birth Certificates and other vital records are issues through the Illinois Department of Public Health. There are several methods for ordering a copy. You will have the option of a full certified copy or for a slightly lower fee, a short form abstract.
The office of the IL Division of Public Records is located in Springfield, Illinois. You can request it in person there and it will be mailed out within two weeks.
You can mail in a request to the Division of Public Records for a new birth certificate with a legible copy of your valid drivers license or state ID. It will be mailed out to you within three to four weeks.
You can send a fax to 217.523.2648 and request a new birth certificate with a legible copy of your valid drivers license or state ID. You can also send your credit card information with the fax and your request will be processed within 5 days.
You can go to the website of the Illinois Department of Public Health and there are links to be able to order a copy online using a credit card. This should also be processed within 5 days.
