Check the state requirements for the certificate, and request or download a certificate application form. Some states may require a sworn statement with the birth certificate application form. Others may ask for a photocopy of a government-issued photo identification, such as a driver's license.
Mail the form and any requested documents to the state office of vital records. There is a cost for this service.
