Fill out the application form to get the birth certificate by mail or in person. You can download and print the application (see link under Resources below). You will need a check or money order for the fee, which is $10.00 per copy. Make the check or money order payable to the Kentucky State Treasurer.
Apply in person if you are in or can visit Frankfort. This is the only location of the Office of Vital Statistics where you can make an in-person application. However, if this is feasible, it's the fastest method, as you will normally be able to pick up the birth certificate the next business day. The OVS is located at 275 East Main Street in Frankfort.
Mail your application to the Office of Vital Statistics. Enclose the completed application form with a check/money order, and mail them to the address listed on the application.
