Determine if you are eligible to get a particular replacement birth certificate in Oklahoma. Birth certificates may be issued to the person who is the subject or to parents, spouses, and some other family members. Legal representatives can obtain a certified copy under some circumstances. Anyone applying for a certified copy of a birth certificate in Oklahoma must provide either a primary form of identification such as a driver's lines, state-issued photo ID or military ID, or passport. If a primary ID is not available, substitute 2 forms of alternative identification, including W-2 forms, utility bills, and paycheck stubs.
Complete the application form. Provide the full name of the person the birth certificate belongs to along with the birth date, location, and parent's names along with you own contact information. You can download an application for a replacement birth certificate on the Oklahoma Vital Records Service website (see "Resources" below).
Apply in person to get a replacement birth certificate in Oklahoma, if you are in or can visit Oklahoma City, Tulsa, Or Macalester. These are the only three locations where the public may get official documents in person. Be sure to bring identification (see Step 1) and allow a minimum of 1 hour for processing of your application. Hours and addresses are listed on the Vital Records Service website.
Apply by mail by sending a completed application form, the required identification document(s) and a check or money order for the fee made out to "Vital Records Service." Processing usually takes about 4 weeks. If you wish to expedite your application, send it by US Postal Service Express Mail and include a prepaid return Express Mail envelope. Expedited applications are usually processed in 48 to 72 hours. Do not use other delivery services like Federal Express. The application will be processed, but will not be expedited.
