How to Obtain a Birth Certificate in Indiana

Birth certificates are a required form of identification if you apply for a passport, obtain a driver's license or register a child for school. The Indiana State Department of Health maintains birth records for all births that occurred in the state since October 1907. To request a copy of a birth certificate, you must be the person listed as being born (the registrant) or the registrant's parent, sibling, spouse, child over 18 years of age, grandparent, aunt or uncle.

      Download a birth certificate application from the Indiana State Department of Health website.

      Photocopy an acceptable form of identification, such as your driver's license. You can find a complete list of acceptable forms of identification on the Department of Health website.

      Complete the application, providing at a minimum the registrant's name, date of birth, place of birth and parents' names. Provide your name, address, relationship to the registrant and the reason for the application at the bottom of the application.

      Mail or bring the application, a copy of your identification and a check or money order for the application fee ($10 as of November 2010) to an Indiana State Department of Health office. Allow three to four weeks for processing of your mail-in request. In-person requests will be fulfilled the same day.
