Download a copy of the fax application for a Wisconsin Birth Certificate from the website provided in the resources column below. Fill out the application as completely as you can. Provide all pertinent information such as year off birth and place of birth.
Fax the completed application to the Wisconsin Department of Health Services and pay the required application fees. See the website listed in the resources section below for the fax number. The Wisconsin Department of Health Services will then search for your birth certificate and retrieve it from any of the 50 states in the United States.
Include information on the application about how you want to receive your birth certificate. It can be sent by first-class mail within one month of application, or you can receive it by United Parcel Service in a couple of days by paying the extra fees.
