Collect the required information for the application, such as your birth date, your mother's full maiden name and the city and county where you were born. Application forms are available online from the state or in person at a New Jersey county registrar's office. Information on both is in Resources.
Present acceptable form of identification with your application, which can include a valid state-issued driver's license or non-driver's license. Have the original forms of identification with you if applying in person or make copies if applying online or via mail.
Pay the application fee of $25 (as of 2010). A check or money order should be made out to "Treasurer, State of New Jersey." You may request multiple copies of your certified birth certificate for an additional $2 each.
Mail your application, fee and proof of identification to the appropriate address (listed on the application form). You can also complete the process online, by phone or by fax using Vitalcheck (see Resources). Vitalcheck will charge an additional fee ($10.95 as of 2010), but you have the option of paying with a credit card and you will receive your certified copy faster--in 15 to 22 days--than if you choose to send your application in writing to the state.
