Fill out the information either online or by hand on Form DS-11 if this is your first U.S. Passport, or the applicant is under 16 years of age or was when the previous passport was issued. You must also use Form DS-11 if you last got a passport more than 15 years ago or your last passport was lost or stolen. Also, use the form if your name has changed and you cannot legally document this change.
Form DS-11 is available on the U.S. Department of State website. Click on " Apply for a Passport," then on "Form DS-11: Application for a U.S. Passport." You will need to supply your name, date of birth, address, Social Security number, travel plans and marriage history.
Submit the Form DS-11 in person at any acceptance facility in New Jersey along with evidence of U.S. citizenship. This evidence can be an undamaged U.S. passport, a certified birth certificate with raised state seal, a Consular Report if you were born abroad or a Certification of Birth. A Naturalization Certificate or
certificate of your citizenship is allowed. A passport acceptance facility can be found at post offices, public libraries and other state, county, township and municipal government offices.The U.S. Passport Service Guide website lists all of the addresses for the passport offices in the United States.Click on the state and select the office nearest you.
Present acceptable identification when you apply. You may use an undamaged U.S. passport, a current valid driver's license, a current city, state or federal government ID, a naturalization certificate or a military and dependent ID that must be current.
Submit a photocopy of the front and the back of the identification used in the previous step. The paper must be 8 1/2 x 11 inches, plain white, with printing on one side only. If you need to, use two sheets of paper to obtain copies of both sides of the ID.
Provide two passport-sized photos that must be identical and taken within the last six months. They must also be in color and have a full face, front view with a plain white or off-white background. The photos must be 2 x 2 inches.
Pay the applicable fee for the passport. Payment may be made with a credit card, check or money order.
