Report your travel emergency. According to Congressman Bennie Thompson's office, you should call the National Passport Information Center at 877-487-2778 and speak to a customer service representative. Explain that you have a life or death emergency. The representative will give you an immediate appointment. Because there are no agency offices in New Jersey, you will need to visit either the New York Passport Agency (located at 376 Hudson Street in New York City) or the Philadelphia Passport Agency (located at 200 Chestnut Street in Philadelphia).
Assemble the required passport documentation. You will need proof of your emergency. You will also need a valid form of photo identification (such as a driver's license), your original birth certificate and two passport photos, according to Congressman Jim Matheson's office. Prepare to bring your tickets and a copy of your travel itinerary, if your travel plans have been finalized. Print and complete Form DS-11 -- Application for a U.S. Passport, and note the appropriate passport fees (see Resources).
Visit the Regional Passport Agency. Make it a point to be on time. You will not be admitted more than 15 minutes before or 15 minutes after your appointment time. After checking in with building security, you will report to the agency's reception desk and provide your Social Security number or appointment confirmation number. The receptionist will give you a queue number. When your number is called, an agent will assist you with your passport request.
