Call the National Passport Information Center at 1-877-487-2778, which connects to an automated appointment system available 24 hours a day, including holidays and weekends. Schedule an emergency passport appointment at the nearest Passport Agency. If you can't find an appointment soon enough, stay on the line to ask a Customer Service Representative for help scheduling an appropriate appointment. Complete Form DS-11, the application form for a U.S. passport. You can complete it online or print it to complete by hand. Do not sign the application until an acceptance agent prompts you to do so at your appointment.
Prepare proof of U.S. citizenship. Acceptable options include a previous, undamaged U.S. passport, a certified birth certificate or a naturalization certificate. Alternatively, you can provide two or more samples of secondary evidence of citizenship, such as a hospital birth certificate, census record, early school records or a doctor's record of post-natal care, each of which is unacceptable when presented alone.
Make a photocopy of the front and back of your identification -- such as a valid driver's license, a current military ID or a naturalization certificate -- on plain white 8 1/2-by-11 inch standard stock paper. You can enlarge the ID in the copy, but the U.S. government prohibits decreasing the size.
Prepare proof that you are set to travel within two weeks. Typically, this documentation is an international flight itinerary with your name on it. Not every passport agency requires this proof. Check with your local passport agency (on the U.S. Department of State website) to see if this is a requirement at your agency.
Take one 2-by-2 inch self-portrait, in color. The photo should have a plain white or off-white background. Face the camera with a neutral expression, meaning lips closed and eyes open. Hats, headphones and non-prescription glasses are unacceptable for the photo. If your prescription glasses have a glare, remove them for the photo.
Arrive at the Passport Agency for your appointment on time. The agency will not admit you 15 minutes before or 15 minutes after your scheduled appointment time, which gives you a window of approximately 30 minutes to enter. Take money, as your agency may require you to pay an expedite fee.
Check in at the information desk. Present your appointment confirmation number or your social security number, as well as proof of your reason for emergency travel.
Take your queue number and sit in the waiting area until officials call your number. Your passport agent will assist you in handing in your passport application and all supporting documents from this point.
