Find a guarantor that meets the conditions listed on the website for Passport Canada or call 800-567-6868 to obtain a statutory declaration in lieu of guarantor (form PPTC 132). A guarantor is someone who can affirm the information given in your passport application.
Print an application form from the website of Passport Canada and fill it out. Three application forms are available: one for children under 16, one for adults over 16 and a simplified renewal form. You can renew if you were at least 16 when you applied for your previous passport, which must be an undamaged, regular passport, valid for five years and issued in Canada within the last six years under your current name.
Locate proof of your Canadian citizenship. If you were born in Canada, submit an original copy of your Canadian birth certificate issued by the province where you were born or an original certificate of Canadian citizenship. Those born outside of Canada must provide one of four certificates: Canadian citizenship, naturalization in Canada, retention of Canadian citizenship or registration of birth abroad.
Provide proof of your identity--not applicable for children. Acceptable documents include a driver's license issued by the state or province, provincial or state health-care card, other government-issued ID card, a federal identification card, military identification, certificate of Indian status, alien registration card, old age security card, government employee ID or a Canadian passport.
Refer to the photo checklist in the application form packet. Submit two identical black and white or color 2 inch by 2.75 inch passport-style photos with your application. On the reverse of one photo, write from top to bottom; the name and address of the photographer, date the photo was taken and "I certify this to be a true likeness of (Your Name)" with a signature of your guarantor.
Pay the fee by writing your Visa, MasterCard and American Express information on the application or obtain a certified check or international postal or bank money order payable to the Receiver General. Fees vary, depending on your age and quantity of passport pages. A schedule of fees is available on the website for Passport Canada.
