Get your Canadian birth certificate or your Certificate of Canadian Citizenship. If you were born in the Quebec province after 1994, only birth certificates issued by the directeur de l'état civil are honored. If you were born in Manitoba, only a birth certificate issued by the Manitoba Vital Statistics Agency is honored. The Certificate of Canadian Citizenship is honored for all provinces.
Provide one document that supports your name, such as a government-issued identification card such as a driver's license, non-driver's license, health care card, military ID and others. (Reference 3)
Get your fees and two photos together. The fees are $97 in Canadian currency for a 24-page passport, $102 in Canadian currency for a 48-page passport. You can pay by certified check, international money order Visa, Mastercard or American Express. Make the check payable to the Receiver General for Canada. (Reference 1)
Find a guarantor to complete and sign the guarantor section of the passport application. The guarantor also must sign one of the photos on the back. Your guarantor must hold a valid five-year Canadian passport, have known you personally for two years, be a Canadian citizen 18 years or older, been 16 years or older when they applied for their current passport, reside in Canada, the United States or Bermuda and be accessible to Passport Canada for verification if needed.
