How to Obtain a United States Passport for a Minor Child

All U.S. citizens regardless of age must have a U.S. passport for international air travel. Beginning on June 1, 2009, all U.S. citizens traveling by land or sea must have a U.S. passport book, passport card or enhanced driver's license to enter the United States from Canada, Mexico, the Bahamas and Bermuda.
The application for a minor's passport has different requirements than applying for an adult's passport. Parents or guardians must be present with the minor, and additional documents must be provided with the application.

      Obtain proof of citizenship. You can use one of the following documents: previously issued passport, certified birth certificate, consular report of birth abroad, certificate of naturalization or certificate of citizenship.

      Obtain proof of relationship to parent(s) or guardian(s). You can use one of the following documents: certified U.S. birth certificate with both parents' names, certified foreign birth certificate with both parents' names, report of birth abroad with both parents' names, adoption decree with adopting parents' names, court order establishing custody, or court order establishing guardianship.

      Provide proof of legal name change if a parent or guardian has a different last name than the one listed on one of the above documents.

      Go to the nearest passport agency or acceptance facility. You can find the nearest office at the Department of State website (see link in Resources). Acceptance facilities are often located in federal, state and probate courts, post offices, libraries and county and municipal offices. Most cities have multiple acceptance facilities. There are only 14 passport agencies nationwide, and all are located in major cities.

      Fill out the DS-11: Application for a U.S. Passport at the office to submit with your application. Both parents must sign this form in front of a passport agent. If one parent cannot come, the parent that is present must provide a notarized statement of consent from the second parent. If one parent has sole custody, that parent must bring a certified legal document proving sole custody.

      Provide identification for both parent(s) or guardian(s) at the passport office. You can provide one of the following identifications: current government employee ID, current military ID, certificate of naturalization, previously issued passport or a current driver's license.

      Provide two photos of the minor that are 2-by-2 inches in size. You can get the photos taken at the passport application site for a fee. You can also get passport photos taken at a FedEx Office location.

      Pay the fee. Refer to the link under Resources for current application fees. A minor's passport is less expensive than an adult passport.
