Supply two photos taken within the last 12 months. Ensure that the name and address of the photographer or studio, and the date the picture was taken, are written on the back of one of the photos. Leave space for the name of the applicant, guarantor's signature and guarantor's statement. The photographer may not use a label for this information.
Complete the Declaration of Guarantor section on the passport application, if you are the guarantor. Be sure to sign it. You must also sign the photo. A guarantor must be someone known to the applicant for two years, live in France or the Principality of Monaco, be available for verification, can confirm your identity and is a dentist, judge, practicing lawyer, magistrate, mayor, medical doctor, notary public, signing officer of a bank or a police officer. If you don't know anyone who meets the qualifications, complete form PPT 132, which you can get at any Passport Canada location or by calling (819) 997-8338, Monday through Friday, 7:30 a.m. to 8:00 p.m. (EST). Legalization fees will be charged and must be converted to euros.
Provide your original proof of Canadian citizenship. This would be your Canadian birth certificate or a Certificate of Canadian Citizenship issued after 1977. If you need a passport immediately and do not have time to get your birth certificate, you may complete forms PPT 113 and PPT 116. You will be issued a temporary passport, valid for a few months.
Pay your fees by personal check or money order. Make them payable to the Canadian Embassy. Provide one document to verify your identity. It must be issued by a territorial, provincial, federal or municipal authority in Canada. If you send originals, they will be returned. If you send copies, your guarantor must sign them.
