Determine if you are eligible to renew your passport by mail. You must meet one of these requirements: your most recent U.S. passport is undamaged, was issued when you were at least 16-years-old, was issued in the last 15 years, or was issued in your current name. If you had a name change and can provide documentation for the change, you are also eligible. If you don't meet any of the requirements, you must apply in-person.
Locate and complete the passport renewal application form DS-82. Sign and date the application form on the first page.
Gather the following application materials: your most recent passport; two passport photos; processing fees (see "Passport Fees" under Resources); and legal documentation such as a marriage certificate or court order that reflects your name change, if applicable.
Mail the completed application form and all application materials in a padded envelope to the National Passport Processing Center, P.O. Box 90155, Philadelphia, PA 19190-0155.
Locate and complete the passport application form DS-11 (see Resources).
Gather documents that prove your U.S. citizenship and identity. Acceptable documents that show U.S. citizenship include a previously-issued U.S. passport or U.S. birth certificate. Examples of identification include your driver's license or U.S. passport. Make photocopies of these documents.
Locate the nearest acceptance facility. Use the Passport Acceptance Facility Search tool or consult the Ohio Passport Acceptance Facility List, which lists all the facilities that process passport applications in Ohio by county (see Resources for both links).
Bring the completed application form DS-11, originals and photocopies of documentation and identification, two passport photos and processing fees to your local acceptance facility.
