Unless you are renewing your passport, you need to apply for your U.S. passport in person. Download and complete Form DS-11 and submit it to your local passport agency.
To apply for a U.S. passport, you must provide evidence that you are a U.S. citizen, such as a previously issued U.S. passport, naturalization certificate, certificate of citizenship, consular report of birth abroad or certified birth certificate.
You'll need a form of identification, a copy of that identification and two passport photos. Acceptable identifications include a previously issued U.S. passport, naturalization certificate, valid driver's license, valid government ID or valid military ID.
The two passport photos you submit must be identical, in color and 2 inches by 2 inches in size. The photos must have been taken during the past six months and show your current appearance.
Adult passports cost between $20 and $95, and passports for minors cost between $10 and $70.
