      Go the Centers for Disease Control & Prevention website's listing of "Where to Write for Vital Records." (See Resources.)

      Note the physical address if you want to request the certificate by mail. Click on the link to that state's department of vital records if you want to request and pay for the certificate via the Web.

      Write a brief letter requesting a certified copy of your birth certificate and enclose the photocopy of your ID with the check or money order for the fee if you are requesting the certificate by mail. Include as much information as possible in your request, including your name, your parents' names, your date of birth, the city and county where you were born, the hospital where you were born and information on how to reach you. Address the letter to the appropriate office and mail.
