Complete form DS-11. You can download this form from the State Department's travel website or you can pick it up at a passport facility. Answer all questions on the form truthfully.
Travel to your nearest acceptance facility or passport agency. You can find a list of the facility nearest you on the State Department's travel website.
Submit your DS-11 form along with proof of citizenship, which includes a certified copy of your birth certificate or your naturalization or citizenship certificate.
Provide proof of identity with a driver's license, naturalization certificate, government-issued ID or current military ID.
Include a photocopy of all your identification documents with your DS-11 form. Include the front and back sides of all documentation.
Pay the appropriate fee. You can find current passport fees on the State Department's website.
Submit one color passport photo. The photo must be 2 by 2 inches and your head must fill 1 to 1 3/4 inches of the space. It must have been taken of how you currently look in the past six months. The photo must be taken on a white background with the person looking straight into the camera with their eyes open and a neutral facial expression.
