Complete the appropriate form. If you are renewing your passport you may use the form DS-82 and if you are applying for the first time it is form DS-11. Be sure to use the appropriate form to ensure timely processing of your passport. Complete the form in dark ink and write neatly. You may also complete the form online and print it.
Sign it in front of a passport official or mail. If you are applying for the first time do not sign your application before you arrive at the post office or other passport processing center. You will be sworn in and then must sign the form in front of the passport official. If you are renewing your passport you do not need to do this. You do need to fill out the correct form and send your old passport with it.
Include the appropriate fee, documents, and photos. Depending on whether you are renewing or applying for the first time, the fees will be different. Also, if you want expedited service this will cost more. Determine the correct fee for your application and include it with your application, documents like birth certificate or marriage certificate, and your 2 passport photos.
Mail your passport to the appropriate processing center. Look on your application to find the correct address where you should send your passport application. The state you live in will affect where you send your passport and the correct address will be on your application.
