Get a copy by going to the Office of Vital Records in person if you are in Nashville. You will need to bring a government-issued ID (driver’s license, military ID or State issued ID) and you will be able to pick up your copy that day or the next. There is a map on the right of the page.
Mail in your request to get a copy of a birth certificate in Tennessee. This is the most convenient method for most people. You can download the application form using the link below. Fill out the form and make a copy of a government-issued ID such as your driver’s license, military ID, or state photo ID. For other forms of identification that are accepted, click on the “government-issued identification” link on the Tennessee Vital Records page (link below).
Mail the form along with the copy of your photo ID and a check or money order made out to "Tennessee Vital Records" to the address on the form. Don’t send cash because they won’t accept it. Allow 3-4 weeks for delivery.
Order by credit card if you prefer by calling (615) 741-0778. You will still need to fax a copy of a photo ID if you choose this option. Alternatively, you can order online through an independent contractor (VitalChek) by choosing the Order Online link below and choose the “Birth” tab at the upper left. If you do order online using a credit card there is an additional fee.
