Read the instructions noted on the appropriate New York birth certificate website.
Copy the required New York identification documents. Those who have a government-issued photo ID may copy this document to prove their identity. People without such photo ID need to copy two proofs of address, such as a letter issued by the government within the last six months, utility bills, bank statements or telephone bills.
Print and fill out the required application form noted on either the New York State or New York City birth certificates website. You will need to provide information such as your full name, date of birth, location of birth and parents' names.
Mail the form, identification and required payment to the appropriate New York birth certificates agency. As of 2009, typical birth certificate fees were $30 to $55, depending on how quickly the request must be met.
Wait to receive your birth certificate in the promised time frame, which is usually one week to 30 days, depending on the option selected. If you encounter an unexpected delay, telephone the appropriate New York agency to inquire on your application status.
