If you or a member of your immediate family was born in Missouri, you may order a certified copy of the birth certificate. Legal representatives can also order a certified copy for certain reasons. If you are ordering by mail or in person at local health agency you'll need to fill out the application form, which is available online (link below). Payment for mail or in person orders must be by check (U.S. banks only) or money order (from a U.S. bank or the U.S. Postal Service).
To get a new birth certificate in Missouri you need the birth name of the person (and the current name, if different) and date of birth. You also need to provide the place of birth (and hospital name if possible) and parents' names. If you are ordering a certified copy for another person you must include your name and relationship to the person whose name is on the birth certificate. You must state the purpose for getting a copy of another person's birth certificate.
To order a certified copy of a Missouri birth certificate in person you can take a completed application to a local public health agency (see link below to find the one you need). To order by mail send the application and a self-addressed stamped envelope (legal size) to the Bureau of Vital Records of the Missouri Department of Health and Senior Services. The mailing address is PO Box 570, Jefferson City, 65102. Enclose a check or money order for the fee ($15 as of 2009). Do not send cash.
