The office is open Monday through Friday from 8 a.m. to 3:30 p.m. There will be a $10 fee per birth certificate, payable by check or money order to the "Kentucky State Treasurer." You will receive the birth certificate the same day.
Order by mail. Download a birth certificate application through the Kentucky Office of Vital Statistics website (see Resources). Fill out the form completely. Enclose a check or money for $10 per birth certificate made payable to the "Kentucky State Treasurer." Mail your application and payment to: Kentucky Office of Vital Statistics, 275 E. Main St. 1E-A, Frankfort, KY 40621. Note that it may take up to 30 days for your birth certificate request to be processed and mailed.
Order by phone. Call (800) 241-8322 Monday through Friday between the hours of 8 a.m. and 4:30 p.m. Have your birth information, mailing address and a major credit card handy. An additional credit card fee will be charged on top of the regular birth certificate fee. Overnight courier service is available for an extra charge.
Order by fax. Download a fax birth certificate application through VitalChek (see Resources). Fill out the application with your birth information and credit card information. VitalCheck will charge a $9 processing fee on top of the regular birth certificate fee. Fax the application to (866) 283-7477. The birth certificate will be sent through regular mail at no charge. UPS Next Day Air service is available for $17.50.
