Download a birth certificate application (see Resources). Fill it out completely.
Present an identification document. A photo ID, such as a driver's license, state ID or passport is acceptable. If you do not have a photo ID, you will need to present two pieces of secondary identification, such as a pay stub and a Social Security card. See resources for a complete listing of acceptable identification. Make a photocopy if you are mailing or faxing your application.
Pay the appropriate fees. The first copy is $17.75. Additional copies are $10 each when ordered at the same time. Write a check for the total made payable to Vital Records, or pay by credit card. If you choose to pay by credit card, a $9 processing fee applies.
Provide proof of relationship or authorization if you are not the person listed on the birth certificate, such as a family member. Birth certificates, marriage certificates, court orders and insurance polices are acceptable documents.
