      Go to the North Carolina Vital Records website, and click on "Requests", then the "Births" section.

      Download the application for a certified birth certificate.

      Fill out the form. You will need to provide your full name, date of birth, mother's full maiden name, father's full name and the place (city or county) of birth.

      Provide your relationship in the space that says "requester". A certified copy of a birth certificate may only be obtained by the person born, or a spouse, sibling, direct ancestor, direct descendant, stepparent or stepchild.

      Mail the form along with a check or money order made out to "North Carolina Vital Records" to the address on the form. Your birth certificate will arrive in 9 to 10 weeks.
