Write down the date,your purpose for requesting a birth record, your mailing address and phone number. Indicate which record you are requesting. If you request a Certification of Report of Birth, indicate how many copies you need. Note that you may only request one Consular Report of Birth Abroad.
Write down your full name at birth, your date and country of birth, your parents' full names and their dates and places of birth. If you were adopted, write down your adoptive name.
Write down some information from the first passport you used to enter the United States. This includes your name as printed on the passport, the date of issuance, the passport number, and the date of inclusion if applicable.
Write down some information from your current passport. This includes your name as printed on the passport, the date of issuance, and the passport number.
Take the request document to a notary public. The document will need to bear your notarized signature. Do not sign the document in advance.
Include your original Consular Report of Birth Abroad if you are requesting a replacement copy of this document. If you have lost your Consular Report of Birth Abroad, prepare an affidavit confirming its loss. This statement will also need to bear your notarized signature.
Include a photocopy of your government-issued ID.
Write a check or money order for the applicable fee. As of 2010, a Consular Report of Birth Abroad and a Certification of Report of Birth are $30. Additional copies of a Certification of Report of Birth are $20 each. Make the check or money order payable to the "Department of State."
