How to Replace a Pennsylvania Photo Identification Card

If you do not have a Pennsylvania driver's license, a photo identification card is a valid document for establishing your identity, age and residence. If you have lost the card, it's important to acquire a new one as soon as possible. You can make your application for a replacement through the Pennsylvania Department of Transportation's Bureau of Driver Licensing.

      Obtain a Form DL-54B: Application for Change/Correct/Replace/Renew a Pennsylvania Photo ID card. This is available for through the Pennsylvania Department of Transportation website and at your local Pennsylvania Driver's License Center.

      Use blue or black ink to fill out Section A and Section B of the form. If you need to make a change or correction to your Photo ID, fill out Section C.

      Bring the form to a notary public. Sign Section D in his presence and have him notarize the signature.

      Include a check or money order payable to "PennDOT" for the correct fee. As of 2010, the fee is $12.

      Mail your application and fee to: Bureau of Driver Licensing, P.O. Box 68272, Harrisburg PA 17106-8272. You can also apply in person at your local Pennsylvania Driver's License Center.
