How to Get a Tennessee ID Card

As a Tennessee resident, you will need to apply for one of the two state identification cards. Potential drivers who provide acceptable proof of identity can apply for an Expiring Identification License. This form of identification needs to be renewed periodically. Intellectually or physically disabled residents qualify for a Permanent Identification License. This form of identification never expires. Applicants under the age of 18 must have a parent or guardian complete the Minor/Teen-age Affidavit and Cancellation form.

      Locate your nearest driver's license testing center. Find information in your county phone directory. Inquire if an appointment is needed to apply for your identification card.

      Take the following documentation to apply for your identification card:

      Proof of citizenship or residency - official birth certificate or permanent resident alien card.

      Primary proof of identity - passport, military identification card or naturalization card.

      Secondary proof of identity - work or school identification card or completed tax forms.

      Proof of name change - marriage certificate or legal name change form.

      Proof of Tennessee residency - utility bill, rental/mortgage contract or letter from an employer.

      Your social security card or sworn affidavit stating that you were never issued a Social Security Number (SSN).

      Teenage affidavit if you are under 18. This must be completed by your parent or guardian.

      If applying for the permanent identification license, you must provide a physician's statement indicating your inability to drive a vehicle presently or at any time in the future.

      Inform the driver's license center that you wish to apply for your state identification card. They will provide the proper forms, which you will need to fill out completely.

      Submit your application and documentation where indicated by customer service. Pay your application fee of $12.50 (USD) by cash or check. Your application and documentation will be checked for errors and verified.

      Have you picture taken. When your information is verified, you will be summoned to take an identification card photo.

      Receive your Tennessee identification card, after a brief wait. All submitted documentation will be returned to you at that time.
