Visit the Kansas Department of Health and Environment office online. (Link at bottom)
Choose which program you want to open and fill out an application with. You can choose either Adobe or Microsoft Word.
Click on How to Order a Birth Certificate above the application form options. Read through the information and choose which option will work best for you. You can choose to send your order by regular snail mail, internet, phone, priority mail (2-3 business days), walk-in or fax.
Fill out the application, print it out and then send it in. Make sure you have all the correct information provided.
