Protect your financial health. If you are requesting a copy of your Social Security card because yours was lost or stolen just reporting your card lost or stolen is not enough.Start by filing a complaint with the Federal Trade Commission. You call also check your Social Security record to make sure your income is being calculated correctly by calling toll-free 1-800-772-1213. Monitor your credit reports and contact the authorities if there are any discrepancies.
Complete an application for a Social Security card. Application Form SS-5 is available for download on the Social Security Administration website. Print on standard 8.5 x11-inch white paper using blue or black ink to ensure the application will not be rejected.
Locate your local Social Security Office. You can find the location of your local Social Security Office by using the Social Security Office Locater on the Social Security Administration website.
Provide proof of citizenship. Under new laws only certain documents are acceptable as proof of U.S. citizenship. Acceptable documents include your U.S. birth certificate, a U.S. passport, a Certificate of Naturalization or a Certificate of Citizenship. Only original documents or copies certified by the issuing agency will be accepted. Therefore, no photocopies are allowed.
Provide proof of identity. Identifying documents must show your name, identifying information about you and a picture. Acceptable documents include a U.S. driver's license, a state-issue identification card for non-drivers and a U.S. passport. If you do not have these documents and cannot readily obtain them then you will need to show alternative documents including an employee ID card, school ID card, health insurance card or a U.S. military ID. As with proof of citizenship only original documents or copies certified by the issuing agency will be accepted.
Turn in your application at your local Social Security Office. After turning in your application and showing the necessary proof of citizenship and identity your replacement Social Security card will be mailed to you.
