Gather the required documents. You will need your birth certificate, documents of changes in civil status (marriage, divorce, change of surname) issued by foreign countries, official documentation that you are in the country legally (a notarized copy of your green card for example if reapplying for a passport while living in the United States), proof of blood type, and a passport application filled out in Romanian using block letters.
Get passport pictures taken. You will need to provide three color photos: one 1.5-inch-by-1.8-inch color photo and two 3.5-inch-by-5-inch color photographs. The photos should have a white background printed on matte paper.
Purchase a $150 (U.S., as of November 2010) money order made out to the Embassy of Moldova. You will also need a postage-paid self-addressed envelope.
Mail all the required documentation, photographs and the fee to the nearest Embassy of Moldova.
