      Complete form DS-82. If you need a second passport because your first one expired or your name changed, then you need to use form DS-82. You can pick this form up at your local post office or you can download it from the Department of State's website. Fill out the form accurately, taking your time and ensuring your writing is legible. You can also fill the form out online and print it.

      Once you have completed the form DS-82 you need to send it to the passport processing center for your state with passport photos, fee and old passport. If you have lost your old passport you will need to report it so it can be remove validation. If you do have your passport you need to send it in. The validation will be removed, hole punched, and returned to you. Any documents you send with your application will be returned to you in most cases as well.
