Get a letter on your company's or organization's letterhead, signed by your supervisor, that has contact information for the organization and clearly states that the second passport is needed for travel to the countries in question.
Make an appointment with the passport office or embassy. Passport offices only accept visitors with an appointment. For a list of passport offices, see Reference 3. For a list of embassies and consulates, see Reference 4.
Present the completed form DS-11, "Application for a U.S. Passport," to the agent.
Present the letter stating the need for a second passport to the agent.
Provide documentation of U.S. citizenship. A current passport is sufficient.
Provide identification. A current U.S. passport is sufficient, so this step will occur at the same time as Step 4.
Provide photocopies of the current passport and any other documentation submitted.
Pay the fee for the issuing of the passport--$135.00 (as of 2010) for persons over 16. Cash or credit cards are accepted.
Provide two identical photographs conforming to the requirements of the U.S. Department of State.
Wait until the new passport is finished. This takes a couple of days, and it may be expedient to ask for it to be mailed to you.
