How to Replace a Lost United States Passport

Your passport is an official government travel document that is essential if you travel over seas and need to re-enter into the United States. It certifies your nationality and identity. If your passport was lost or stolen you need to take immediate action, report it missing and fill out necessary paper work to acquire a new one.

Things You'll Need

  • Certified birth certificate
  • Valid drivers license
  • Form DS-11
  • Form DS-64
  • Two 2 x 2 inch passport photos

      Report your lost Passport to the U.S. Department of State immediately by calling 877-487-2778. If your passport was lost or stolen overseas, contact the nearest U.S. Embassy or Consulate (see Resources section).

      Print and complete forms DS-11( Application for a U.S. Passport) and DS-64 (Statement Regarding a Lost or Stolen Passport). Both forms can found online or obtained from any Passport Acceptance Facility (see Resources section).

      Take your completed forms and supporting documentation to a Passport Acceptance Facility or Passport Agency. Applicants must show proof of U.S. Citizenship by providing the clerk with a certified birth certificate and other photo identification such as a valid drivers license and two identical 2 x 2 inch passport photos. Applicants can find Passport Acceptance Facilities that provide photographs on site or nearby by using the Passport Acceptance Facility search page and selecting the additional search criteria: photo on-site or photo nearby (see Resources section).

      Pay applicable fees. As of 2010, there is a $75 application fee and $25 execution fee charged for an adult Passport Book. Standard processing time for applications takes approximately four to six weeks. Your application status can be tracked online within five to seven days of submitting your application (see Resources section).
