Refer to the installation guidebook you received upon being assigned to a new duty station and locate your installation travel office. Request Form DS-71, the Affidavit of Identifying Witnesses and the No-fee Passport Application 1056 DOD for completion.
Visit the JAG (Judge Advocate General) office near your post accompanied by a witness to have Form DS-71 notarized. Your witness will be required to complete the witness portion on the form by providing their personal information, and the applications portion will be for you to fill out.
Set an appointment at your designated installation travel office and bring your passport materials. Your passport photos will be taken on-site.
Complete Form DS-82 and sign it in the presence of office personnel. Prepare a copy of your military orders, passport forms and passport photos, and provide a stamped, self-addressed envelope. Mail the travel documents to the National Passport Processing Center (see Tips) and allow 2 to 5 days to have your No-fee Passport delivered in the mail.
