Documents Required for Issuing a Passport

The United States government has specific guidelines for issuing passports. The first time passport applicant has to apply in person at a passport agency or facility, bring two 2x2 passport photos, pay a fee and supply identifying documents.
  1. Application

    • A DS-11 Passport Application which includes social security information needs to be completed. The application remains unsigned until the applicant is directed to do so at the agency.

    Citizenship

    • Proof of citizenship is required. A certified birth certificate, certificate of citizenship, certificate of birth abroad or a naturalization certificate is acceptable.

    Identification

    • One item of primary identification and a photocopy of both sides of the item are necessary. Acceptable identification items are a valid drivers license, a current military or government id or a naturalization certificate.

    Minors

    • Parents or guardians need to accompany those sixteen and under who are applying for a passport. In addition to the standard items, they also need to present evidence of a relationship to the adults with them such as a birth certificate, adoption decree or court order establishing guardianship or custody.

    Parents and Guardians

    • When accompanying minors, parents and guardians must also present a primary identification document and a photocopy to establish their own identities.
