A DS-11 Passport Application which includes social security information needs to be completed. The application remains unsigned until the applicant is directed to do so at the agency.
Proof of citizenship is required. A certified birth certificate, certificate of citizenship, certificate of birth abroad or a naturalization certificate is acceptable.
One item of primary identification and a photocopy of both sides of the item are necessary. Acceptable identification items are a valid drivers license, a current military or government id or a naturalization certificate.
Parents or guardians need to accompany those sixteen and under who are applying for a passport. In addition to the standard items, they also need to present evidence of a relationship to the adults with them such as a birth certificate, adoption decree or court order establishing guardianship or custody.
When accompanying minors, parents and guardians must also present a primary identification document and a photocopy to establish their own identities.
