Required Documents for a U.S. Passport Application

U.S. citizens need a passport to enter most other countries. You also need one to re-enter the United States. The process for obtaining a passport is detailed, so plan accordingly.
  1. Application

    • All new applicants must submit Form DS-11: Application For A U.S. Passport at a post office or other approved passport acceptance facility. Renewals handled by mail utilize Form DS-82.

    Primary Evidence

    • You must be a U.S. citizen to receive a passport. To prove you are a citizen, you must present a certified birth certificate, a naturalization certificate, a consular report of birth abroad or certification of birth, or a certificate of citizenship.

    Secondary Evidence

    • You can submit a combination of early public records as evidence of citizenship. These include a baptismal certificate, a hospital birth certificate, an early school record and a doctor's record of post-natal care.

    Identification

    • You must present identification. This can be a previously issued, undamaged passport; a valid driver's license; or another government-issued ID.

    Minors

    • Minors must have the consent of both parents. If both parents are unable to apply in person, a notarized statement of consent must be submitted for the absent party, unless sole custody can be established and adequately documented.

    Other Requirements

    • At the time of your application, you must present two passport-size photos of yourself, all applicable fees and photocopies of all identification.
