All new applicants must submit Form DS-11: Application For A U.S. Passport at a post office or other approved passport acceptance facility. Renewals handled by mail utilize Form DS-82.
You must be a U.S. citizen to receive a passport. To prove you are a citizen, you must present a certified birth certificate, a naturalization certificate, a consular report of birth abroad or certification of birth, or a certificate of citizenship.
You can submit a combination of early public records as evidence of citizenship. These include a baptismal certificate, a hospital birth certificate, an early school record and a doctor's record of post-natal care.
You must present identification. This can be a previously issued, undamaged passport; a valid driver's license; or another government-issued ID.
Minors must have the consent of both parents. If both parents are unable to apply in person, a notarized statement of consent must be submitted for the absent party, unless sole custody can be established and adequately documented.
At the time of your application, you must present two passport-size photos of yourself, all applicable fees and photocopies of all identification.
