A form DS-11 (Application for a U.S. Passport) and DS-64 (Statement Regarding a Lost or Stolen Passport) are needed to reapply for a replacement passport. These forms can be found on the U.S. Department of State website, travel.state.gov/passport. Forms should be filled out--but not signed--in advance of being turned in at the local Passport Agency.
When applying for a replacement passport, a certified birth certificate, naturalization certificate or certificate of citizenship is required. An identification card--valid driver's license, naturalization certificate or military/government ID--is also required. The Passport Agency will require a photocopy of the front and back of the ID to be presented. Two identical (2-inch-by-2-inch) color photos are needed to reissue a replacement passport.
When not being used, a passport should be kept in a secure location such as a home safe. A passport should never be left in a car or unsecured in a hotel room. When traveling, make a copy of the passport and take it along. This will help expedite the process of replacing a lost passport.

The U.S. Department of State recommends that a lost passport be reported immediately. The National Passport Information Center can be contacted at 1-877-487-2778. Representatives are available from 8 a.m. to 10 p.m. (EST) Monday through Friday. The office is closed on all federal holidays.
