* The patient must be severely obese, with a BMI of 40 or higher.
* The patient must have tried and failed to lose weight through other methods, such as diet and exercise.
* The patient must be at high risk for developing serious health problems, such as heart disease or diabetes, if they do not lose weight.
* The patient must be willing to undergo a comprehensive evaluation to determine their suitability for gastric bypass surgery.
* The patient must be willing to follow a strict post-operative diet and exercise plan.
If a patient meets all of these criteria, their gastric bypass surgery will be covered by Michigan Medicaid.
