Step 2: Applicant Information
- Fill in the applicant's name and rank.
- Enter the applicant's Air Force specialty code (AFSC) and duty title.
- Indicate the applicant's experience level (Basic, Intermediate, or Advanced).
Step 3: Training Information
- Specify the training location.
- Provide the training organization or unit's name.
- Enter the training course title and number.
- Indicate the training dates (start and end dates).
- Fill in the total number of training hours.
- Describe the training objectives and any special topics covered during the training.
- Mention any specific certifications, qualifications, or skills acquired as a result of the training.
Step 4: Course Completion Status
- Check the appropriate box to indicate whether the applicant successfully completed the training.
Step 5: Supervisor's Endorsement
- Your immediate supervisor should sign and enter their name, rank, and position.
- The supervisor should provide a brief statement (max. 200 characters) confirming the applicant's successful completion of the training and their satisfactory performance.
Step 6: Approving Authority
- The approving authority (a senior officer or commander) should sign and enter their name and rank.
- The approving authority should also provide a brief statement (max. 200 characters) endorsing the applicant's training completion and acknowledging the value of the training to the Air Force.
Step 7: Distribution
- Make copies of the form as required and distribute them to the appropriate personnel or agencies.
It's important to ensure accurate and complete information is provided on AFTO Form 350 to document and record an individual's training experience within the Air Force.
