Complete an Affidavit of Loss form. Head to any DOL office to obtain one, or download it dol.wa.gov/forms/420040.pdf.
Sign the form, along with all registered owners in front of a DOL clerk. If mailed, all signatures must be notarized.
Pay applicable fee of $9. A sub-agent fee of $12 is assessed if you pay at a DOL office. Otherwise, send the form with a check to: Department of Licensing, P.O. Box 9909, Olympia, Washington 98507-8500.
Await receipt of new title. It should arrive via mail within eight to 10 weeks.
Complete a Vehicle Certificate of Ownership Application. Pick this up from a DOL office, or download it at dol.wa.gov/forms/420001.pdf.
Locate the receipt of the trailer, showing sales tax was paid in full. If you are not able to provide proof, a tax-based, on-average market value is assessed.
Sign the completed form, along with all other registered owners, in front of a DOL clerk. If mailing the form, all signatures must be notarized.
Pay registration fee of $11.65. An additional sub-agent fee of $5 assessed if paying at a DOL office. Otherwise, send the form, documents and check to: Department of Licensing, P.O. Box 9909, Olympia, Washington 98507-8500.
