Write a letter requesting a report. The Los Angeles Police Department (LAPD) requires the name and address of the victim on the police report, an insurance number, type of report (such as traffic accident or assault), date and location where the incident took place and an incident report number, if you have it.
Include a check or money order for $23, payable to the LAPD, to cover a records search. The fee is non-refundable.
Mail the written request to the Records and Identification Division of the Los Angeles Police Department. The address is:

Wait for a response. Because the LAPD serves a heavily populated area, it may take a week or more to receive your report.
