Each applicant must complete an application, which is available on the Fiji Embassy website. You must use uppercase block letters in English and include your signature. If the applicant is unable to read or write, or is disabled, the left thumbprint serves as a valid replacement for the signature. If the applicant is under the age of 16, a parent or guardian must complete and sign the application. The Fiji Embassy suggests that the application is sent by FedEx to the following address:

Each applicant must submit their birth certificate along with their application. If the applicant is married, he should also submit a marriage license. Children who have been adopted must include their adoption papers with the application, and school-age children must also submit report cards. In addition to their birth certificates, adults must submit an additional form of identification, such as an FNPF (Fiji National Provident Fund) membership card, drivers license, bank or business card, or green card.
A payment of FJ $74 is required with the application for a new passport. It can be paid by cash, postal or money order, or check enclosed with the application.
Applicants must submit three copies of the same portrait photo along with the application. The photo must be certified by a witness. Acceptable witnesses are a notary public, member of parliament, head or deputy head of a government department, mayor, magistrate, barrister and law solicitor, bank officer, justice of the peace, minister of religion, administrative officer, medical or dental practitioner, education officer, assistant education officer or head teacher, officer or non-commissioned officer at the Fiji Military Forces, or member of any town council, township board or provincial council.
