The child's parents must complete a DS-11 form, before the process of obtaining a passport can begin. This form MUST be signed by both parents in front of a certified passport agent, and will not be accepted if it was signed prior to arrival at the passport agency.
Proof that the child is a legal United States citizen is part of the application process. Some of the acceptable forms of identification include an expired passport, an original or a notarized copy of a birth certificate or a naturalization certificate or certificate of citizenship.
The adults applying for a passport for their child must be able to prove that they are, in fact, the actual parents or guardians of the child. Some forms of proof include the child's official birth certificate with both parents' names listed, an adoption decree with both parents' names listed or documentation from the court that states custody or guardianship.
Acceptable identification must be presented during the application process. This includes a valid U.S. passport, a naturalization certificate, a valid driver's license, a government employee ID card or a military ID card.
Both parents must appear in person during the application process, and both must sign the DS-11 form stating that they give their permission for the minor to obtain a passport.
Just as required with adults, a child must appear at the passport processing agency office with two acceptable and identical passport photos.
