Documents Needed to Obtain a US Passport

Passports are identification issued by the State Department. You can apply at a Passport Acceptance Facility or, if you need your passport in a rush, at a Regional Passport Authority.
  1. Application Form

    • The application is a Form DS-11 and you can download it from the Department of State website. After filling it out, do not sign the form until you are instructed to do so at the Acceptance Facility.

    Proof of Citizenship

    • U.S. citizenship can be proved with a current U.S. passport, government birth certificate, certificate of citizenship or naturalization certificate.

    Proof of Identity

    • You can prove your identity with a current U.S. passport, naturalization certificate, driver's license, government ID or military ID. If you use an out-of-state driver's license, you must present another form of ID.

    Photos

    • You must have two copies of your head shot to be put in your passport. The photos must be in color and measure 2-by-2 inches.

    Fees

    • If you are 16 or older, the cost of the passport is $100; if you are 15 or under the cost is $85.
