To renew your passport, mail your current passport, application form DS-82, two passport photos and a $110 fee to:

You can access the DS-82 form on the Travel.State.Gov website by clicking "Passports" and then "Forms." The DS-82 requires that you provide your full name, address, phone number, Social Security number, date and place of birth, sex, physical description, occupation, emergency contact and travel plans.
It generally takes about six weeks for passport applications to be processed. To expedite a passport renewal, which will allow you to receive it within three weeks, include an additional fee of $60 in your application materials.
If your name has changed, you must provide documentation, such as a marriage certificate or divorce decree. If your passport has been lost or damaged, you must apply for a renewal in person. This requires a different form, the DS-11. Renewals of lost or stolen passports also require the DS-64 form.
