When you misplace a passport, before getting a new one, you need to notify Passport Services of the loss. You can do this by filling out Form DS-64, which you can easily retrieve, download and print through the U.S. Department of State's official website. The form asks for personal details such as your full name, gender, Social Security number and birthday. Apart from personal information, you also must specify details regarding your lost passport, including the passport number, time and location of issue, and circumstances regarding the loss, for example: "I left the passport on a train en route to the airport."
Form DS-11 is a general passport application. If your passport was lost, you also must fill out this form. Find it on the Department of State website. Some of the information you must provide on the form includes emergency contact details, your eye and hair color, your reasons for traveling outside of the country and your home phone number and home address.
To get a new passport, you need to provide several items, such as two straight-on passport photographs of your face that are 2 inches by 2 inches and proof of American citizenship. Acceptable proofs of citizenship include a birth or a naturalization certificate. Payment of a passport processing fee also is required.
To replace a passport, you must visit your nearest passport facility, many of which are available at post offices, but may be limited to specific days of the week. Once you submit all of your forms, it usually takes six weeks or less to receive the new passport in the mail. If you urgently need to leave the country quickly, you can request expedited service for an additional fee.
